Service Overview
 
   
 
Your healthcare provider has prescribed TelehealthLink’s services to monitor potential health problems. TelehealthLink’s devices record vital signs to help physicians more quickly diagnose, treat and improve your health conditions.

1. Enroll

Your healthcare provider will determine which type of services is best for you. He or she will prescribe specific medical devices as well as a TelehealthLink communication gateway and enroll you in the service.

2. Set-Up

During your appointment, a representative from your healthcare provider office will show you how to setup and use TelehealthLink remote patient monitoring (RPM) system. They will demonstrate the system features and configure your RPM services for you. Your healthcare provider will then give monitoring devices to you, or TelehealthLink will send the monitoring package to your home. Within 24 hours of enrollment, a TelehealthLink Customer Service Representative will call you to discuss your service, or you can call our 24/7 toll free customer service line at 1-800-600-8138 for assistance.

3. Monitor

Take vital sign measurement as your healthcare provider has recommended. To help improve the effectiveness of disease management, it is important to take your vital sign measurement at the recommended time during your prescribed service period.

4. Record

Each of TelehealthLink monitoring package has been designed to record and upload your vital signs automatically. If your vital sign readings are out of the pre-set normal zone, an alarm will be generated and delivered to your healthcare providers and/or family members instantly. All vital sign measurements can be displayed in a web browser in real-time mode.

5. Report

TelehealthLink creates vital sign reports using the data collected from your monitoring devices with detailed data and charts. Your physician and healthcare providers use these detailed reports to diagnose and manage your diseases and health conditions.

6. Return

When you have finished with your prescribed service, please return the whole monitoring package including medical devices and communication gateway to TelehealthLink in the pre-paid envelope provided to you or contact TelehealthLink to schedule the package pick-up.
